ONLINE BOOKING TERMS & CONDITIONS

PLEASE READ CAREFULLY PRIOR TO REQUESTING AN APPOINTMENT

By requesting an appointment with Skin + Tonic Medical & Laser Aesthetics (“the Practice”), you acknowledge that you have read, understand, and agree to the following Terms and Conditions.

OUR COMMITMENT

Our mission is to provide ethical, safe, and medically appropriate aesthetic care. We are committed to honest education, individualized treatment planning, and prioritizing client safety above all else.

APPOINTMENT REQUEST & CONFIRMATION

• All online appointment requests are subject to approval
• Appointment confirmation will be sent via email approximately one (1) week prior to the scheduled appointment
• Appointments are not guaranteed until confirmation is received

The Practice specializes in customized treatment planning, which allows services, pricing, and recommendations to be determined based on individual medical evaluation and aesthetic goals.

PRE-TREATMENT & POST-TREATMENT REQUIREMENTS

Clients are solely responsible for reviewing all Pre/Post treatment instructions no less than 7–10 days prior to their scheduled appointment.

Pre/Post care instructions are available on the Practice website and are required for treatment safety, medical clearance, and optimal outcomes.

Failure to follow required instructions may result in treatment postponement or cancellation without refund.

DEPOSIT & PAYMENT AUTHORIZATION

• All online bookings require a minimum $50 non-refundable deposit at the time of scheduling
• Deposits are applied toward provider consultation time, medical history review, and treatment clearance
• A valid credit card must be maintained on file at all times

By submitting your appointment request, you expressly authorize Skin + Tonic Medical & Laser Aesthetics to charge the card on file in accordance with these policies.

NO-SHOW & LATE CANCELLATION POLICY

The Practice reserves appointment times exclusively for each client. Missed appointments result in lost provider time, staff preparation, and medical resources.

Cancellations / Rescheduling

• A minimum of 48 hours’ notice is required to cancel or reschedule an appointment
• Cancellations or reschedules made within 48 hours of the scheduled appointment will result in:
– Forfeiture of the $50 deposit
– An additional $50 non-refundable cancellation/reschedule fee

No-Show Appointments

• Failure to arrive for a scheduled appointment without notice may result in a charge of $50 up to 100% of the scheduled service fee
• Charges will be applied to the credit card on file as athorized by your submission for appointment time reservation. 

Clients with a history of no-shows or late cancellations may be required to prepay future services or may be restricted from future bookings.

MEDICAL EVALUATION & PROVIDER DISCRETION

• A complete medical history and consultation are required prior to treatment
• All services are rendered only after medical clearance
• The Practice’s licensed medical providers reserve the right to refuse or postpone services based on professional medical judgment

NO GUARANTEE OF RESULTS / NO REFUNDS

• Aesthetic medicine involves both predictable and unpredictable outcomes
• Results are not guaranteed
• No refunds are provided for treatments, packages, or products, whether rendered or not
• Additional treatments required to achieve desired results are the client’s responsibility

All aesthetic procedures carry inherent risks and potential complications. Should additional or corrective services be required, these services are not included unless otherwise stated in writing.

LICENSURE & COMPLIANCE

• Skin + Tonic Medical & Laser Aesthetics operates as a HIPAA-compliant medical facility
• All services are performed by state-licensed, trained professionals
• The Practice operates in accordance with applicable Michigan laws and medical delegation standards

PARKING INFORMATION

The Practice is located off I-96 at the Spencer Road Exit.

• Turn right at Grand River & Main Street
• Immediately turn left onto the one-way street
• The first lot marked Private Parking is designated for our clients
• Walk up the hill — the Practice is the third door on the right

By requesting an appointment through our online booking system, you confirm that:

• You have read and understand these Terms & Conditions
• You agree to all payment, cancellation, and medical policies
• You authorize charges to the card on file in accordance with these policies
