To book an appointment, we require a valid credit or debit card to be placed on file for all clients.
No Call/No Show appointments : If you book an appointment and do not show, we will automatically charge the card on file 50% of the appointment value on the day of your missed appointment.
Consultations are $100 with Luis Muniz RN, Stephanie Brown NP and Dr. Jim Garza
Consultations are $50 with all other providers.
****Our consultation fees are charged the day of your consult.
****100% of your consultation fee will be applied to your treatment within a 3 month period from the date of your consultation.
Payment: 100% of payment is expected at the time of service.
Pure Med Aesthetics accepts payment in the forms of cash, check, credit or debit card and care credit. however care credit cannot be used as your deposit
Refund Policy: ALL TREATMENT SALES ARE FINAL. Before a service is performed please consider all the required protocols and side effects. We are committed to client satisfaction and are available to answer any questions or concerns you may have in regards to the services and or pricing we offer before your purchase.
Cancelled Appointments within 24 hours notice: Scheduled appointments represent a cost to us, and to other clients who could have been seen in the time set aside for you. We require a 24 hour notice for canceling or rescheduling of any appointment. You will be charged a $50 dollar cancellation/rescheduling fee and you are agreeing to this charge with the credit card on your file.
Services Policy: Pure Med Aesthetics has the right to refuse treatment and/or dismiss a client from any service at any time. I also understand that I may not be a candidate for certain medical services and it is at the full discretion of the medical provider to determine whether I am a candidate for any service provided.
If you have purchased a treatment, you may not receive a refund for the unused portion of services however it may used as a credit toward any other service or retail item.
Only one discount at a time can be used towards a service/retail item.
Photographic documentation will be taken for your chart and purpose of following your treatment with us. I consent for any photographs or video to be used for my medical records.
